A paradigm shift that extends classical means and capability. Genomic and genetic information is part of patient profile and may suggest //predispositio//n of the individual toward disease manifestation (diagnosis and prognosis). On the other hand, analysis on the //-omics// levels extends traditional clinical data models. Thus medical decision making extends in two sides: on the one side to include @@color(blue):genotypes@@, and on the other to include (when appropriate) //omics// findings - the @@color(blue):phenotypes@@.
Yet integration is not easy simply because at both ends (genotype, phenotype) the amount of available data is immense, and complexity of processes is high. At the up-stream (or research) level scientists may be interested in the investigation between genotype-phenotype information to form //associations// and //patterns// of disease and //susceptibility// indices. At the middle-stream (industrial level) technology and service providers are interested in embedding research advances into concrete products and services via intelligent devices. The complex clinico-genomic environment is occupied by a set of problems that challenge the traditional and long-standing coupling between medical decision making and Artificial Intelligence:
* //How to cope with the ''huge amounts'' of genomic/genetic bio-data produced by high-throughput// (HTP) //platforms?// Targeting whole genome studies and using HTP platforms we are now able to capture and measure thousands of transcriptomic (e.g., genes) to millions of genetic (e.g., single nucleotide polymorphisms) features in the whole genome. Here the call targets the utilisation, customization and deployment of high-performing computational environments, i.e., ''Grid''-enabling infrastructures;
* //How to cope with the ''heterogeneous'' nature of complex clinico-genomic data?// On one hand, identification (by population studies) of genetic polymorphisms may be linked directly to the genetic background of individuals; on the other hand, gene-expression studies capture the genomic profile of individuals and target a higher transcription-machinery level that reaches the underlying //regulatory// mechanisms in the cell (gene regulatory networks and metabolic pathways). Efficacy and reliability of models post the need to consider all different bio-resources in the respective data analysis processes. Here the call targets the utilisation of ''semantic integration'' technology and ''interoperable'' solutions as realised by //standard information/data-models and ontologies//, //Web//-//Services// and //Scientific Workflows//;
* //How to ''correlate'' the classical clinical profile of individuals (e.g., biochemical, histological, drug-response etc) with their genetic and genomic background?// The target is to uncover and reveal reliable ''//clinico-genomic associations//'' of high diagnostic, prognostic and therapeutic value. Here the need is forwarded towards the devise of //intelligent data-analysis// processes that take advantage, '//hybridize//' and '//amalgamate//' established bio-knowledge sources: the //''bio-knowledgeable''// data analysis endeavor. The key and enabling technology is presented by ''data-mining'' and ''knowledge discovery'' algorithms, operations, systems, tools and services. In addition, research findings should be well-documented, indexed and traceable. Here ''argumentation'' and ''recommendation'' systems present the key technology to realize.
